If you have a life-threatening illness causing emotional or physical distress, call the Palliative Care Team. Team members will counsel patients and families when medical decision-making becomes difficult. They will listen and provide support through all stages of an illness.

The Palliative Care Team supports patients and families to:

    heart Explore options for the next step in managing your illness
    heart Discuss your condition with your doctor
    heart Understand Advance Directives
    heart Manage pain or other distressing symptoms
    heart Cope with feelings of being confused and overwhelmed
    heart Discuss spiritual needs
    heart Improve communication with your family about your illness
    heart Provide education and support regarding available resources

The Palliative Care Team supports Physicians through:

    heart One-on-one consultations with patients and families
    heart Professional education and collaboration concerning palliative care, symptom       management and complex decisions
    heart Psycho social and spiritual support to patients and families as they face complex issues and decision-making

The Palliative Care Team partners with Fairchild Medical Center staff:

    heart To Facilitate the transition of patients and families from disease management to comfort care at the end of life

The Palliative Care Team supports our community by providing:

    heart Educational forums concerning end-of-life issues
    heart Information about resources
    heart Support for those living with life-threatening illness
